Racers/Team Captainns have the option to print and complete waiver before check-in. Waiver will not be accepted electronically. Printed waivers and survey be collected on Saturday morning.
The PDF file contains 1 waiver and 1 hotel survey. The waiver cannot be altered in any way including the size (8 ½ X11). PRINT SINGLE SIDE ONLY. DOUBLE SIDED PRINTING NOT ACCEPTED. Each racer and team captain must complete their own waiver form and survey.

Page 1 – USCA Waiver – At the top of page write Team Member or Team Captain and include boat Number. Sign waiver where it says Participant Signature and then completely fill out Items 1-7. If a team member is under 18, then complete the bottom section.
